Statement of Economic Interests

Any Statement of Economic Interests form provided by and filed with the Cook County Clerk's office discloses the information required by law and will meet the statutory filing requirement for candidates.

Candidates can print out the Statement of Economic Interests form below and deliver it to the Clerk's downtown office (69 W. Washington, Fifth Floor, Chicago, IL 60602) or any of the Clerk's suburban courthouse office locations (Rolling Meadows, Skokie, Bridgeview, Maywood and Markham).

Please complete the Candidate Contact Form and file with your nomination papers or email the form to colleen.gleason@cookcountyil.gov. Do NOT bind with the nomination papers.

Candidate Filing

Candidates must file their nomination papers during regular business hours (8:30 a.m. to 5 p.m.) between Nov. 25 and Dec. 2. However, the Clerk’s office will be closed for Thanksgiving on Thursday, Nov. 28. Due to a countywide furlough day, the Clerk’s office will not accept petition filings on Friday, Nov. 29. Withdrawal of Candidacy

To withdraw your candidacy, please complete the Withdrawal of Candidacy form, have it notarized, and file it with the office where you submitted your original nominating petitions. If you filed your petitions with the Cook County Clerk's office, you should deliver or mail your form to the Clerk's downtown office: 69 W. Washington, Fifth Floor, Chicago, IL 60602. Emails and faxes are not accepted.

Important Dates for the March 18, 2014 Consolidated Election

  • September 3, 2013: First day to circulate petitions for established party candidates (90 days prior to last day of filing).
  • November 25–December 2, 2013: Candidate filing period for established party candidates.
  • December 9, 2013: Last day to file objections to nominating petitions established party candidates.
  • December 11, 2013: Last day to conduct lottery for the November 25 – December 2 filing period filing period (conducted within 9 days following the last day of petition filing).
  • December 16, 2013: Last day to file petitions for public question (certain exceptions apply).
  • December 30, 2013: Last day to adopt a resolution or ordinance for binding/advisory public question.
  • January 9, 2014: Certification of ballot by local election authorities (68 days before consolidated election)
  • January 16, 2014: Last day to file a Declaration of Intent to be a write-in candidate with the appropriate election authority or authorities (61 days prior to the election)
  • February 6, 2014: First day of any registered voter within the confines of the U.S. to make application by mail or in person to the E/A for an official ballot. (40 days prior to primary)
  • February 18, 2014: Last day for registration or transfer of registration within the office of the election authority. Registration closes 27 days preceding an election.
  • February 19, 2014: First day for grace period registration and grace period voting in the office of the election authority or at a location designated for this purpose by the election authority. Grace period voting may be done by mail at the discretion of the election authority. (begins at the close of registration and continues until the 7th day prior to the election)
  • March 3, 2014: First day for early voting at the office of the election authority or at a location designated by the election authority for this purpose. (begins 22nd day preceding election and extends through the 5th day prior to an election)
  • March 15, 2014: Last day for grace period registration and grace period voting in the office of the election authority. Grace period voting may be done by mail at the discretion of the election authority. (7th day prior to election)
  • March 15, 2014: Last day for early voting at the office of the election authority or at a location designated by the election authority. (5th day prior to the election)
